Transcripts
Domestic Transcripts
You will need to request that one set of transcripts from all courses taken beyond high school** be sent to SOPHAS. SOPHAS will need to receive these transcripts no later than December 2, 2011; they should be postmarked no later than November 24, 2011. It is recommended that you request these transcripts as soon as possible. The mailing address for SOPHAS is:

**This requirement includes original study abroad transcripts as well as transfer credit transcripts. Exceptions will only be granted if the course name, number, grade, and credit hours appear on your home institution transcript. If all four pieces of information do not appear on your home institution transcript, you will need to obtain original study abroad and transfer credit transcripts.
International Transcripts
Send to SOPHAS one official transcript from each institution attended beyond high school. Official transcripts must be submitted DIRECTLY from the institution in a sealed envelope with the signature/stamp of the institution. Materials required for the SOPHAS application should be sent to:

You do NOT need to submit third-party evaluations. Academic records should be issued in their original languages accompanied by English translations (usually prepared by your university or governmental agency, unless the institution issues original documents in English). Click for more instructions about international transcripts. Do not mail any other materials to the School of Public Health unless expressly advised to do so. Official transcripts from international applicants must be postmarked by November 24, 2011. Unsolicited materials will be destroyed.
All Applicants:
It is crucial that you send every transcript taken beyond high school to SOPHAS. If there are any missing transcripts, your application will be held indefinitely at SOPHAS and the School of Public Health will be unable to review it. Please ensure that any college you have attended, no matter how briefly, sends a transcript to SOPHAS at the above address. You should verify in your SOPHAS application portal that each transcript has arrived.
UC Berkeley will require official transcripts at matriculation, so please be prepared to send another set of official transcripts if you are admitted and choose to attend Berkeley.
