For information about the On-campus/Online M.P.H. Professional Degree Program, please see the program's website for information. Applications to this program are accepted three times a year. The On-campus/Online M.P.H. Professional Degree Program does not use the SOPHAS application service.
SOPHAS Application: Schools of Public Health Application Service
Fill out the SOPHAS application, available online in September of 2011. This application is due December 2, 2011, at midnight Pacific Standard Time. You must pay the required fee for your application at the time of submission. If you would like to apply for a fee waiver, please do so early by following these instructions.
Entering coursework:
You will need to list on the SOPHAS application all of the coursework you have completed since high school, including any college courses taken during high school. When entering your college coursework, please list all courses from the first half of your degree (e.g. freshman and sophomore years) as "lower division," and the second half of your degree (e.g. junior and senior years) as "upper division." If you completed your undergraduate degree in a less traditional fashion (in 3 years or in 5+ years), you should list your final 60 semester units or 90 quarter units as upper division and the remainder as lower division. Designating the your desired degree and area of concentration:
You will use the School Designations section to declare the program and area of concentration. Click on "Add/Update Designations" in the top left hand corner of the screen. Choose "University of California Berkeley" from the first drop down menu. Then, choose your desired degree and area of concentration from the second drop down menu. Be sure that you designate the same degree and area of concentration on your SOPHAS and UC Berkeley Graduate Applications.
Answering the UC Berkeley-specific additional questions:
Once you choose your degree and area of concentration under the School Designations section, you will need to answer a few questions specific to the UC Berkeley School of Public Health. The link to the questions is available on your list of school designations, by clicking on "Additional Questions" in red.
This is where you will enter your Personal History Statement (please note that the text box limit is 4000 characters!) and provide other supplemental information of interest to the admissions committee.
Checking your application status:
It is your responsibility to regularly check your application status to be sure your materials are received by December 2, 2011. You can check to see if your transcripts, letters of recommendation, and official test scores have been received by SOPHAS. You can also check to see if your application has been sent to us by SOPHAS. More information about the SOPHAS application status menu. Applicants are responsible for ensuring that their completed applications (with three letters of recommendation, GRE and/or TOEFL scores, and ALL official transcripts sent to SOPHAS) are sent to UC Berkeley. We will not be able to review any incomplete applications.
