Please note: The application deadline for Fall 2012 admission to on-campus programs has now passed. Check back this summer for application information for Fall 2013.

For information about the On-campus/Online M.P.H. Professional Degree Program, please see the program's website for information. Admission to this program are accepted three times a year.

 

The UC Berkeley Graduate Application for Admission & Fellowships (JAZZEE)

Fill out the UC Berkeley Graduate Application for Admission & Fellowships 2011, available online in early September 2011, and due December 2, 2011 at midnight Pacific Standard Time.

If you want to be considered for financial aid, you must fill out the fellowships portion of the UC Berkeley Graduate Application.

You will be required to upload an unofficial transcript in order to apply; you do NOT need to send an official transcript at this time.

International applicants: In addition to uploading your transcript, make sure to upload a degree certificate (if the degree you earned is not listed on your transcript) and an English translation of both the transcript and the degree certificate.

You should also make a note of your Doc ID #; you will need this number to complete your SOPHAS application. Please note you will not receive an email confirmation as indicated on the SOPHAS application. Your Berkeley Graduate Application ID number is in the upper right-hand corner of your application. If you have already submitted you can log back in to find this number.

You must pay the required fee for this application when you submit it. If you would like to apply for a fee waiver, click here for instructions.

Are you having trouble finding your area of concentration and degree program on the UC Berkeley Graduate Application?

For professional degree programs (M.P.H., Dr.P.H.), choose "Public Health" as your Program, and either "Public Health (M.P.H)" or "Public Health (Dr.P.H.)" as your Proposed department and degree program. You do not need to designate an emphasis; you will provide this information on your SOPHAS application.

For academic degree programs (M.A., M.S., Ph.D.), choose your area of concentration (Biostatistics, Epidemiology, Environmental Health Science, Health Services & Policy Analysis, or Infectious Diseases) as your Program and as your Proposed department and degree program. You do not need to enter an emphasis.

For concurrent degree programs (M.P.P./M.P.H., M.C.P./M.P.H., M.J./M.P.H.), choose either "Public Policy and Public Health" or "City and Regional Planning and Public Health" as your Program and the same for your Proposed department and degree program. Then, under emphasis, choose your desired area of concentration (for M.C.P./M.P.H. only).
